For back sleepers it is recommended the use a firm and large pillow and a big fluffy pillows and also to use a big fluffy pillow under your knees to reduce the pressure on the sciatic nerve of your lower back.

As for pregnant women, sleeping on their backs will mean more pressure on their already overburden backs, causing more backaches.
